Organisational Regulations of the healthcare entity

§ 1. General provisions
- These Regulations set out the organisational structure, procedures and principles governing the operation of the healthcare entity managed by ArtLife Sp. z o.o., with its registered office in Gdańsk (80-398), at 23 Obrońców Wybrzeża Street, entered in the Register of Entrepreneurs of the National Court Register (Krajowy Rejestr Sądowy – KRS) maintained by the District Court Gdańsk-Północ in Gdańsk, 7th Commercial Division of the National Court Register, under KRS number 0000249525, Tax Identification Number (NIP) number 5832930309, National Business Registry Number (REGON) 220175048, with share capital of 300 500 PLN, fully paid up.
- The healthcare entity operates under the name Centrum Okulistyczne ArtLife (hereinafter referred to as “ArtLife Ophthalmologic Centre”).
- Registered office and place where healthcare services are provided: 23 Obrońców Wybrzeża Street, 80-398 Gdańsk.
- The ArtLife Ophthalmologic Centre operates on the basis of:
– generally applicable provisions of law, including:
a) the Act of 15 April 2011 on Medical Activity;
b) the Act of 6 November 2008 on Patients’ Rights and the Patient Ombudsman;
c) the Act of 5 December 1996 on the Professions of Doctor and Dentist;
d) the Act of 28 April 2011 on the Healthcare Information System;
e) the Act of 10 May 2018 on Personal Data Protection;
f) Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data;
– entry in the Register of Entities Performing Medical Activities maintained by the Pomeranian Voivode;
– these Regulations (hereinafter referred to as the “Regulations”). - The ArtLife Ophthalmologic Centre provides only healthcare services that are medically justified and aimed at preventing diseases, preserving, restoring and improving patients’ health.
§ 2. Objectives and tasks of the ArtLife Ophthalmologic Centre
- The objective of the ArtLife Ophthalmologic Centre is to provide outpatient healthcare services in the field of ophthalmology. The services are provided in accordance with current medical knowledge, with respect for patients’ rights, the principles of professional ethics and applicable legal regulations.
- The tasks of the ArtLife Ophthalmologic Centre include, in particular:
1) ensuring high quality of healthcare services provided through the use of modern and recognised treatment methods;
2) continuous improvement of the knowledge and professional skills of the staff, while adhering to the principles of professional ethics;
3) using high-quality medical equipment subject to regular technical supervision and meeting all quality standards required by law;
4) conducting information and educational activities relating to eye health protection;
5) undertaking preventive healthcare activities.
§ 3. Organisational form and structure of the ArtLife Ophthalmologic Centre
- The ArtLife Ophthalmologic Centre operates through an organisational unit named Specialist Outpatient Clinic.
- The structure of the Specialist Outpatient Clinic includes the following organisational units:

Specialist outpatient treatment 23 Ophthalmology - The tasks of the Specialist Outpatient Clinic include, in particular:
a) organising and providing outpatient healthcare services in the field of ophthalmology;
b) ensuring continuity of the diagnostic and therapeutic process;
c) carrying out preventive and educational activities relating to the protection of eye health;
d) cooperating with other entities performing medical activities to the extent necessary to ensure the proper treatment of patients. - The tasks of the Ophthalmology Clinic include, in particular:
a) performing specialist diagnostic eye examinations;
b) carrying out diagnostic examinations and assessing patients’ suitability for procedures;
c) providing conservative treatment for eye conditions;
d) monitoring patients following procedures performed;
e) referring patients for further diagnostic assessment or specialist treatment where medically justified. - The tasks of the Diagnostic and Treatment Room include, in particular:
a) performing ophthalmic and oculoplastic procedures offered by the ArtLife Ophthalmologic Centre;
b) preparing patients for procedures and providing post-procedure care;
c) operating and performing regular checks of the medical equipment used. - Healthcare services and auxiliary tasks within the ArtLife Ophthalmologic Centre are carried out by:
a) the person managing the ArtLife Ophthalmologic Centre, i.e. the Managing Director appointed directly by the Company’s Management Board;
b) doctors practising within individual professional practices or as part of healthcare entities cooperating with the ArtLife Ophthalmologic Centre on the basis of civil law contracts (B2B);
c) optometrists cooperating with the ArtLife Ophthalmologic Centre on the basis of civil law contracts;
d) nursing staff performing tasks under civil law contracts (service contracts or professional practice arrangements);
e) administrative and support staff cooperating with the ArtLife Ophthalmologic Centre under employment contracts or civil law contracts. - The individual organisational units, as well as the persons and entities cooperating with the ArtLife Ophthalmologic Centre referred to in paragraph 6 above, are required to cooperate in order to ensure the efficient and effective operation of the ArtLife Ophthalmologic Centre in terms of diagnostic and procedural activities, postoperative care, and administrative and operational matters.
§ 4. Method of managing organisational units
- The function of the manager of the ArtLife Ophthalmologic Centre is performed by the Management Board of ArtLife Sp. z o.o.
- The manager of the ArtLife Ophthalmologic Centre manages and directs its activities and is authorised to represent it. The manager is responsible for ensuring that the ArtLife Ophthalmologic Centre operates properly and in compliance with applicable laws and regulations, and represents the ArtLife Ophthalmologic Centre externally.
§ 5. Organisation of the provision of healthcare services
- The ArtLife Ophthalmologic Centre provides paid outpatient specialist healthcare services in the field of ophthalmology.
Healthcare services are provided during the operating hours of the ArtLife Ophthalmologic Centre, in accordance with the appointment schedule established by the person managing the ArtLife Ophthalmologic Centre:
– Monday to Thursday: 09:00–17:00;
– Friday: 09:00–16:00;
– the ArtLife Ophthalmologic Centre is closed on Saturdays, Sundays and public holidays. - Healthcare services are provided by persons practising medical professions and holding qualifications appropriate to the type of services provided.
- Patient registration may be completed in person, by telephone or by email. A contact form, email address and telephone numbers are available on the ArtLife Ophthalmologic Centre’s websites.
- The scope and price list of services are available at the registered office of the ArtLife Ophthalmologic Centre and on its websites: artlife.com.pl and artlifelaser.pl.
- The ArtLife Ophthalmologic Centre may refuse to provide a healthcare service or discontinue further treatment in cases provided for by law, in particular where a patient, their legal representative or accompanying persons seriously violate the rules of conduct applicable within the ArtLife Ophthalmologic Centre, unless the patient’s condition requires immediate provision of healthcare services due to a threat to life or health.
- In the event of an immediate threat to a patient’s health or life, the staff shall provide the necessary emergency medical care in accordance with current medical knowledge. Where necessary, the patient shall be referred to an appropriate hospital or Hospital Emergency Department.
- Healthcare services are provided in accordance with applicable laws and regulations, the principles of professional ethics and the internal procedures of the ArtLife Ophthalmologic Centre.
§ 6. Medical records
- The ArtLife Ophthalmologic Centre maintains, stores and provides access to medical records in accordance with the Act of 6 November 2008 on Patients’ Rights and the Patient Ombudsman, as well as the implementing regulations issued pursuant to that Act.
- Medical records are maintained in paper or electronic form. The ArtLife Ophthalmologic Centre may maintain or reproduce records simultaneously in both forms in order to ensure data security and continuity of diagnostic and procedural care. Each form of maintaining medical records is carried out in a manner ensuring appropriate protection and confidentiality of the personal data and health data contained therein.
- Medical records are made available to the patient, their legal representative or a person authorised by the patient.
- Medical records are also made available to other entities and authorities entitled to access them under the Act of 6 November 2008 on Patients’ Rights and the Patient Ombudsman.
- Access to medical records provided to the patient, their legal representative or an authorised person is free of charge and is not subject to any fees, regardless of the form, scope or frequency of requests submitted.
§ 7. Patients’ rights and obligations
- Healthcare services are provided with respect for patients’ rights as defined in the Act of 6 November 2008 on Patients’ Rights and the Patient Ombudsman and in other applicable legislation.
- In particular, the patient has the right to:
a) healthcare services provided in accordance with current medical knowledge;
b) confidentiality of information relating to their health condition and the course of their treatment;
c) receive clear and comprehensible information about their health condition;
d) access medical records relating to their health condition;
e) submit comments and complaints regarding the healthcare services provided, in accordance with the principles set out in § 8 of these Regulations. - Information on patients’ rights is made available at the place where healthcare services are provided, in written form, in a visible and generally accessible location.
- Before healthcare services are provided, the patient is required to provide medical staff with all information and explanations necessary for the proper provision of those services.
- The patient is obliged to:
a) comply with the provisions of these Regulations;
b) follow the recommendations of medical staff relating to the treatment process;
c) respect the rights of other patients and the staff of the ArtLife Ophthalmologic Centre;
d) respect the property of the ArtLife Ophthalmologic Centre.
§ 8. Procedure for receiving and handling complaints
- A patient who believes that their rights have been violated while receiving healthcare services has the right to submit a complaint in writing, verbally at the reception desk or electronically to the email address of the ArtLife Ophthalmologic Centre. Complaints are handled by the Managing Director.
- A complaint should include, at a minimum: the identifying details of the person submitting the complaint, contact details enabling a response to be provided, a description of the event or circumstances to which the complaint relates, as well as the request or the outcome sought by the complainant, where the complainant specifies one.
- A complaint shall be reviewed without undue delay, and no later than within 30 days from the date on which it is received by the ArtLife Ophthalmologic Centre. The ArtLife Ophthalmologic Centre informs the patient of the outcome of the complaint using the contact details provided in the submission.
§ 9. Terms of cooperation with other entities performing medical activities
- In order to ensure the proper and continuous provision of healthcare services, the ArtLife Ophthalmologic Centre cooperates with other entities performing medical or diagnostic activities.
- The cooperation referred to in paragraph 1 is carried out in accordance with generally applicable laws and regulations, the agreements concluded between the parties, and with respect for patients’ rights.
§ 10. Final provisions
- These Regulations shall enter into force on 1 July 2026.
- In matters not regulated by these Regulations, the generally applicable provisions of law shall apply, in particular the Civil Code, the Act on Medical Activity, the Act on Patients’ Rights and the Patient Ombudsman, and implementing regulations issued pursuant to those Acts.
